My work focuses on adults, age 18 and older, and on those individuals who are interested in and ready to explore the root causes of their difficulties, and are ready to explore ways to create a more satisfying life for themselves.

FEES

My current fees are $190 for an initial assessment, and $125 for a 50-minute session of therapy. For those who do not have insurance, or choose not to use it for reasons of privacy, it may be possible to agree on a lower fee.

As insurance plans change frequently, it is always a good idea to contact your insurance company with the questions below. I am very willing to see you as an in-network provider (if I am contracted with your plan), an out-of-network provider (if I am not contracted with your plan), or on a private-pay basis. For those using insurance, my office will submit claims on your behalf.

  1. What are my benefits for "outpatient mental health" services (psychotherapy)?

  2. Is there a deductible ?

  3. If there is a deductible: have I met my deductible for the current year yet?

  4. What is my co-pay/co-insurance per visit?

  5. Am I limited to a certain number of sessions per year?

  6. Is Carolyn J. Heineman, PhD (a licensed psychologist) an in-network provider for my plan?

  7. If not, what is the difference in my deductible and my copay between in-network and out-of-network providers?

  8. (If relevant) Where can I find a list of in-network providers for my plan?

  9. Is there anything I or the provider need to do prior to, or immediately after, my first visit? (eg, pre-authorization, referral from physician)
